INFORMATION CLAUSE
Pursuant to Art. 13 sec. 1 and 2 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46 / EC (GDPR), we provide the following information:
1. Personal data administrator and contact details:

2. Processed data
The administrator may process the following personal data: name, surname, company name, company address and address data, e-mail address and telephone number, identifier for the messenger preferred for contact, NIP number, REGON number, image, ID card number, passport number , data from the driving license and vehicle registration certificate, bank account number.
3. Purposes of personal data processing and legal basis for processing
Personal data may be processed for the following purposes: if a contract is concluded, personal data will be processed in order to perform the contractual provisions. The legal basis for data processing is Art. 6 (1) (a) of the GDPR. b GDPR (processing is necessary for the performance of a contract to which the data subject is party or to take steps at the request of the data subject prior to entering into a contract); in order to fulfill the legal obligations incumbent on the Administrator, i.e. considering possible complaints, issuing invoices and accounting documents (legal basis: Article 6 (1) (c) of the GDPR); in the event of being on the Administrator's premises subject to video surveillance, personal data will be collected in order to ensure the safety of the Administrator's employees and associates, protect property and keep secret information, the disclosure of which could expose the Administrator to damage (legal basis: Article 6 (1) (f) of the GDPR) ; personal data may be processed for the purpose of sending commercial and marketing information of the Administrator by e-mail and by phone. The legal basis for data processing is Art. 6 sec. 1 lit. a GDPR, (consent of the data subject or, respectively, Article 6 (1) (f) of the GDPR – the Administrator's legitimate interest in connection with the contract between the parties).
4. Data recipients and other entities
Your personal data is or may be transferred to entities cooperating with the Administrator, including contractors participating in the performance of the contract, subcontractors, and entities servicing the Administrator, e.g. IT, accounting and legal service providers.
5. Data storage period
The period of processing your personal data depends on the purpose for which it was collected and on the applicable law. Personal data processed in connection with: the conclusion and performance of the contract for the time necessary for the proper performance of the contract, and then until the expiry of the limitation period for any claims, but not less than 6 years from the end of your company's cooperation with UNIQ LOGISTIC Sp. z o.o. sp. k. with its seat in Łódź; issuing accounting documents – for the period required by the provisions of the tax law and the Accounting Act; pursuing or defending claims, especially in the event of any court dispute – for the time until the final conclusion of the proceedings and the expiry of the limitation periods specified by law. Personal data from video monitoring, i.e. image, date and place of recording, are stored for a period of 60 days. Personal data collected for the purpose of sending commercial information and marketing activities – for an indefinite period, until the consent is withdrawn or an objection is raised.
6. Consent
In accordance with the GDPR, we would like to inform you that in the case of processing personal data based on the consent given, all consents to the processing may be withdrawn at any time without affecting the lawfulness of the processing that took place until the consent was withdrawn.
7. Profiling
When processing your personal data, we do not make decisions in an automated manner, in each case we make decisions with the participation of our authorized employees. We also do not perform profiling consisting in collecting information about the behavior of individual persons and their automatic analysis.
8. Obligation to provide data
Providing data is not mandatory, but necessary to take appropriate steps prior to the conclusion of the contract and its implementation. Providing data and consenting to marketing and commercial activities is voluntary and does not affect the conclusion and performance of the contract.
9. Rights of the data subject
On the basis of and subject to the limitations of Art. 15-22 GDPR, you have: the right to request the administrator to access personal data, the right to rectify, delete or limit processing, the right to object to the processing, the right to transfer data, the right to withdraw consent at any time, the right lodge a complaint with the supervisory body of the Personal Data Protection Office.
10. Data transfer
We store the collected personal data in the European Economic Area ("EEA"), but they may also be transferred to a country outside this area and processed there. Each operation of transmitting personal data is performed in accordance with applicable law. If the data is transferred outside the EEA, UNIQ LOGISTIC Sp. z o.o. sp. k. based in Łódź uses standard contractual clauses and the privacy shield as security measures in relation to countries for which the European Commission has not found an adequate level of data protection.
